Pricing

 

Independent Physician Evaluation  -  $750

(comprehensive assessment including whole health concerns, review of full history & data as provided and validated with testing tools or 3rd parties, and creation of ONE relevant Independent Physician's Report of Evaluation)

Subsequent Hourly Rate  -  $450

(for additional ongoing care after initial Independent Physician Evaluation)

Legal Forms & Paperwork  -  $300 per hour (physician time)

(note: time is NOT prorated)
